Naper Grove Vision Care, P.C. ("Naper Grove") is an Illinois-based vision and eye-care practice. The lawsuit alleges that during a targeted cyberattack on Naper Grove's computer systems in May 2025, certain files containing private information were accessed. According to the court-approved notice, those files may have contained personal information such as names combined with Social Security numbers and other non-public personally identifiable information. Who Qualifies?
The Court has defined the Settlement Class as all individuals whose Private Information may have been impacted by the Data Incident. Excluded from the class are Naper Grove's directors and officers, governmental entities, and the Judge assigned to the case together with that Judge's immediate family and court staff.

How Much Can You Get?
Every class member can enroll in credit monitoring, and can also claim one of two cash options. You may claim Cash Payment A or Cash Payment B, but not both.
Credit Monitoring — one year of CyEx Medical Shield Complete, which comes with $1 million of medical identity theft insurance and monitors for healthcare insurance ID exposure, Medical Record Number (MRN) exposure, and unauthorized Health Savings Account (HSA) activity, with access to a fraud resolution agent if something suspicious happens. Enrollment codes were sent to class members by email or in the mailed Postcard Notice, and coverage becomes active once the Court grants final approval. If you no longer have your enrollment code, you can request it from the administrator through the official settlement website.
Cash Payment A — Documented Losses — reimbursement of up to $1,000.00 for actual, documented, unreimbursed out-of-pocket losses caused by fraud and fairly traceable to the Data Incident, incurred between May 24, 2025 and September 18, 2026. Covered examples include fees for credit reports, credit monitoring, or freezing and unfreezing your credit; the cost to replace IDs; and postage to contact banks by mail. You must submit proof such as bank statements or receipts; self-prepared notes may support other proof but are not enough on their own, and you cannot claim expenses that a third party has already reimbursed.
Cash Payment B — Alternate Cash — instead of Cash Payment A, a one-time pro-rata cash payment. Naper Grove will establish a $50,000.00 Settlement Fund, which is divided pro rata, equally, among all class members who submit a valid Cash Payment B claim. Because the payment is pro rata, the exact amount depends on how many people file; the administrator will post an estimated amount on the settlement website once it is available. You do not have to provide any proof or explanation to submit a valid Cash Payment B claim.
How to File a Claim
The deadline to submit a Claim Form is September 18, 2026. The fastest way to file is online at the official settlement website, Naper Grove Settlement.com. To start the online claim, you sign in with the Login ID and PIN printed on the notice that was sent to you. If you cannot locate your Login ID and PIN, you can request them through the official settlement website.
If you prefer, you can download a printable Claim Form and mail it so that it is received (or postmarked, depending on the method) by September 18, 2026. On the paper form, the Login ID field is optional, so you can file the alternate pro-rata cash (Cash Payment B) by providing only your name and contact information. If you are claiming Cash Payment A for documented losses, complete the losses section, sign the attestation under penalty of perjury, and include supporting documentation for each expense.
On the Claim Form you also choose how to be paid — PayPal, Venmo, Zelle, a prepaid e-Mastercard, or a physical check mailed to your address. Submit only one Claim Form per person, and do not file a claim if you have excluded yourself from the settlement.

If you do nothing, you remain in the Settlement Class and give up your right to sue Naper Grove over the Data Incident, but you will not receive any cash payment. To receive a payment, you must submit a valid Claim Form by September 18, 2026. Benefits are distributed only after the Court grants final approval and any appeals are resolved.
Attorneys' Fees and Service Awards
The Court appointed Gary M. Klinger of Milberg PLLC and Kenneth J. Grunfeld of Kopelowitz Ostrow P.A. as Class Counsel. Class Counsel will ask the Court to approve up to $190,000.00 for attorneys' fees and reimbursement of litigation costs, and service awards of up to $2,500.00 for each of the class representatives. Those amounts are paid by Naper Grove, and class members are not charged separately for Class Counsel's services.
