Dove Healthcare Management Services, LLC, a Wisconsin-based provider of skilled nursing and rehabilitation care, assisted living, and palliative care services, has agreed to a class action settlement to resolve litigation over a July 2024 data breach. The case is Miranda Meredith, et al. v. Dove Healthcare Management Services, LLC, pending in the Circuit Court of Eau Claire County, Wisconsin.
According to the lawsuit, an unauthorized party accessed the Dove Healthcare network in July 2024 and may have obtained personal information belonging to patients and employees. The complaint alleges Dove Healthcare failed to adequately protect that information. Dove Healthcare denies the allegations and any wrongdoing, and no court has found that the company did anything wrong. The two sides agreed to settle to avoid the cost and risk of continued litigation.

What Happened?
Dove Healthcare experienced a cyberattack involving its information systems on or around July 6, 2024. A forensic investigation determined that an unauthorized actor accessed files that could have contained personal information. Dove Healthcare later began notifying affected individuals.
Roughly 16,255 people were affected. The information involved varied by person and may have included names, Social Security numbers, dates of birth, driver's license numbers, medical treatment and diagnosis information, medical history, medical record numbers, health insurance information, and full-face photographs. If you received a notice letter from Dove Healthcare about this incident, your information was among the data the company identified as potentially affected.
Who Qualifies?
The settlement class includes people who were notified by Dove Healthcare that their personal information may have been impacted by the July 2024 data security incident. In practical terms, if Dove Healthcare mailed or emailed you a breach notice about this incident, you are a class member and may file a claim.
You will need the unique ID and PIN printed on your notice to log in to the claim portal, so keep that notice handy. If you believe you were affected but cannot locate your ID or PIN, you can request help through the official settlement website's contact page.
How Much Can You Get?
The settlement offers class members a choice between two cash options, plus credit monitoring that everyone can claim.
Documented loss reimbursement (Cash Payment A). You can claim up to $3,000 for documented, unreimbursed losses fairly traceable to the breach. This can include losses from identity theft or fraud, professional fees, the cost of credit monitoring or a credit freeze you purchased, bank or other fees, and similar related expenses. It can also include up to three hours of lost time spent dealing with the breach, valued at $20 per hour (a maximum of $60) — but lost time is only recoverable if you also submit a valid documented-loss claim. You must provide supporting documents showing the loss and the date it occurred.
Alternative cash payment (Cash Payment B, estimated ~$50). Instead of the documented-loss reimbursement, you can choose a pro rata cash payment estimated at about $50. No documentation is required for this option. The amount may decrease depending on how many claims are approved, so that total cash payments do not exceed the $150,000 cap set in the settlement agreement.
Two years of credit monitoring. In addition to Cash Payment A or B, every class member can also claim two years of one-bureau credit monitoring that includes up to $1 million in identity theft insurance coverage. No documentation is required to select this benefit.
Is Proof Required?
Yes. Filing online requires you to log in to the claim portal using the unique ID and PIN printed on the notice Dove Healthcare sent you, so the page is marked proof required. A claimant who never received a notice, or who discarded it, cannot file online without first requesting that information through the official settlement website.
Beyond the ID and PIN, the documentation needed depends on which option you choose. The estimated $50 alternative cash payment and the two years of credit monitoring require no supporting documents. The up to $3,000 documented-loss reimbursement does require documentation — receipts, bank or credit card statements, or similar records showing the loss and that it was tied to the breach.

Attorneys' Fees and Service Awards
Class Counsel will ask the Court to approve attorneys' fees and costs of up to $212,500, and the class representatives may each request a service award of up to $1,500 for their role in bringing the case. These amounts are subject to Court approval, and the Court may award less than requested. The figures are set out in the settlement documents available on the official settlement website.
