What Changed Recently?

The settlement website and claim process opened after the court's September 29, 2026 preliminary approval order. The case, In re Park Dental Data Breach Litigation, arises from unauthorized activity in Park Dental employee accounts that PDG, P.A., doing business as Park Dental, became aware of on or around January 23, 2024. Park Dental sent notices to affected people in August 2024. The plaintiffs alleged negligence and violations of the Minnesota Health Records Act, among other claims. Park Dental denies wrongdoing, and the court has not decided who is right.

Park Dental operates more than 50 dental offices in Minnesota and western Wisconsin. This settlement is separate from a different incident that Park Dental Partners, Inc. disclosed in 2026, which is the subject of its own lawsuits.

Who Qualifies?

The settlement class is all living individuals in the United States whose private information was potentially compromised in the 2024 data security incident. Park Dental's directors, officers and agents, government entities, and the judge and court staff are excluded.

According to the notice, the information potentially affected included names, medical information, Social Security numbers, driver's license numbers, dates of birth, and financial account or payment card information. For the vast majority of affected people, though, only an internal Park Dental identification number that cannot be used anywhere else was exposed. That matters for the $60 payment described below.

How Much Can You Get?

Each class member can choose one of two cash options, and can add credit monitoring to either:

• Documented losses, up to $2,500. Reimbursement for unreimbursed out-of-pocket losses more likely than not caused by the incident, such as certain bank fees, postage, gas for local travel, fees for credit reports or credit monitoring bought between December 6, 2024 and the claim deadline, and fraud or identity theft losses. The administrator considers whether a loss happened on or after February 22, 2024.
• Lost time, up to $60. $15 an hour for up to four hours spent responding to the incident. This option is limited to class members whose Social Security number and driver's license number were both affected, according to Park Dental's records.
• Credit monitoring. One year of Cyex Financial Shield Complete, with at least $1,000,000 in identity theft protection insurance, which the notice values at $179.40. Every class member can claim it.

There is no settlement fund and no overall cap: Park Dental pays approved claims directly, and approved benefits will not be reduced pro rata no matter how many people file. Attorneys' fees and costs of up to $300,000 and service awards are paid separately and do not reduce class member benefits. Under the preliminary approval order, money from uncashed checks goes to a Minneapolis–St. Paul nonprofit rather than back to Park Dental.

What Proof or Notice ID Is Required?

Filing online requires the claim number printed on the settlement notice. Class members without a claim number can still take part by downloading a personalized claim form from the settlement website, which asks only for contact information, and mailing it in.

The $2,500 option requires reasonable documentation, such as receipts, credit card or bank statements, invoices or phone records; self-prepared documents alone are not enough. The $60 lost-time option requires an attestation of the time spent, and eligibility is checked against Park Dental's records. Credit monitoring requires an email address. Every claim is signed under penalty of perjury.

What Is the Deadline?

Online claims are due by 11:59 p.m. Central Time on December 28, 2026. Mailed claims must be postmarked by December 28, 2026. The deadlines to opt out of the settlement or to object to it are also December 28, 2026. Each class member may file only one claim.

How Do You Take Action?

Claims are filed through the official Park Dental Data Security Settlement website, run by the settlement administrator, Verita Global. The site offers three routes: file online with a claim number, download a personalized form with a claim number to print and mail, or request a personalized form without a claim number to print and mail. Filing is free.

What Happens Next?

Judge Joseph R. Klein will consider final approval at a hearing by video conference on February 1, 2027, at 9 a.m. Central Time. If the settlement is approved and no appeal delays it, benefits will be sent after the settlement takes effect and the administrator finishes reviewing claims. No payment date has been set. Paper checks must be cashed within 90 days of issue.

Questions

Is this the same as the 2026 Park Dental Partners data breach?

No. This settlement covers the incident involving Park Dental employee accounts that Park Dental became aware of around January 23, 2024, with notices sent in August 2024. A separate incident that Park Dental Partners, Inc. disclosed in 2026 is the subject of its own lawsuits and is not part of this settlement.

Who can claim the $60 lost-time payment?

Only class members whose Social Security number and driver's license number were both affected, according to Park Dental's records and the administrator's review. They can claim $15 an hour for up to four hours by attesting to the time spent. The notice says that for most affected people, only an internal Park Dental identification number was exposed.

Will payments be reduced if many people file?

No. The settlement has no overall cap, and the notice says approved benefits will not be reduced pro rata based on the number or value of valid claims. Attorneys' fees of up to $300,000 and service awards are paid separately by Park Dental.

Can a claim be filed online without a claim number?

The online claim form requires the claim number from the settlement notice. Class members without one can download a personalized claim form from the settlement website by entering contact information, then print and mail it, postmarked by December 28, 2026.
