Cash Compensation:

How Much Can I Get Paid?
The cash payment will be calculated by dividing the Net Settlement Fund (after deducting the cost of Privacy Shield services) by the number of valid cash compensation claims. Additionally, you can enroll in one year of Privacy Shield, a data protection and monitoring service.
How Do I File a Claim?
You must submit a claim form online or by mail postmarked no later than December 4, 2024. Claim forms can be found on the official settlement website with the links below or by contacting the official DaVita pixel class action admins. You will be asked for a Member ID. Check Your Notice: Look for your Class Member ID on any notice you received either by mail or email from the DaVita class action administrators.
