A proposed class action settlement has been reached with Family Medicine Centers (FMC Services) over a July 2022 data security incident that potentially exposed the personal information and protected health information of more than 266,000 adults and children. The lawsuit alleged that FMC failed to adequately protect that information from the cyberattack.
FMC denies any wrongdoing and the court has not decided who is right. Both sides agreed to a $2.15 million settlement fund to avoid the cost and uncertainty of continued litigation. Who Qualifies?
You may qualify if you received notice that your personal information or protected health information may have been involved in the July 2022 Family Medicine Centers (FMC Services) data security incident. The settlement class covers the individuals identified by FMC as potentially affected, reported to be more than 266,000 people, including both adults and children.

How Much Can You Get?
Eligible class members choose one of two cash options and may also add data monitoring:
• Cash Payment A — Documented Losses: reimbursement of up to $5,000 for reasonable documented out-of-pocket costs related to the data incident (for example, bank fees, postage, and telephone, correspondence, or receipt records supporting the expense). You must elect this option on the claim form and attest under penalty of perjury, and submit reasonable third-party or contemporaneous documentation. Personal certifications or affidavits alone do not count as documentation.
• Cash Payment B — Alternate Cash: instead of Cash Payment A, a flat alternative cash payment estimated at about $75 with no documented-loss proof required. The actual amount depends on the funds remaining in the settlement fund after other payments.
• Medical Data Monitoring (adults) / Minor Monitoring (minors): in addition to Cash Payment A or B, class members may claim two years of CyEx data monitoring at no cost. For adults this includes one-bureau credit monitoring, dark web scanning, security-freeze and victim assistance, $1,000,000 in identity-theft insurance with no deductible, and access to fraud-resolution agents. The minor product adds monitoring for the child's Social Security number, dark web scanning, public-record traces, and fraud-resolution access.
You may choose only one cash path (A or B). Because benefits are paid from a $2,150,000 settlement fund, cash amounts may be adjusted pro rata depending on the number of valid claims filed.
How to File a Claim
The fastest way to file is online through the official settlement website. You can also request and mail a paper claim form. To file, you will generally need information from your notice (such as a Notice ID or Claim ID, if one was provided) along with any documentation supporting a loss claim.

Important Deadline
The deadline to submit a claim is August 31, 2026 (online or postmarked). Class members who want to opt out of, or object to, the settlement must do so by August 17, 2026. The final approval hearing is scheduled for September 15, 2026 at 9:00 a.m. CT in the 320th District Court, Potter County, Texas, though the date and time are subject to change. Beware of Scams
Legitimate settlement administrators do not ask you to pay a fee to file a claim or to receive a payment. File only through the official settlement website and never share more information than the claim form requires.
