• Credit and medical monitoring: three years of single-bureau monitoring that includes real-time credit-file monitoring, dark-web scanning with notification of potential misuse, public-record monitoring, identity-theft insurance with no deductible, and access to fraud-resolution agents.
• Ordinary losses: reimbursement of up to $150 with documentation, for out-of-pocket expenses such as bank fees, phone charges, and fees for credit reports, credit monitoring, or other identity-theft protection products purchased between August 2022 and August 27, 2026.
• Extraordinary losses: reimbursement of up to $2,500 with documentation for losses traceable to the incident, such as actual identity theft, supported by records like identity-theft reports, affidavits, police reports, or correspondence with financial institutions.
• Lost time: compensation for time spent dealing with the incident.
Claims for ordinary and extraordinary losses require supporting documentation; handwritten receipts alone are generally not enough. Benefits are paid from the $900,000 fund and may be adjusted depending on the number of valid claims and the settlement terms.
