How Much Can I Get Paid?
Each qualified class action member who files a timely claim form will receive a settlement fund of up to $60 from the total $4,485,000 settlement fund. However, if the dollar value of approved and valid claims exceeds the amount, the $60 settlement payments may be reduced to a pro rated amount, distributed equally and fairly.
How Do I File a Claim?
To file a claim, you must be complete and submit a valid claim form. You can download a claim form on the settlement website, request a Claim Form by calling the settlement administrator directly.
The claim form must be completed fully and accurately signed under penalty of perjury and timely submitted by December 23, 2024, either by U.S. mail or online on the settlement website.
Do I Need Proof to File a Claim?
Any claim form must completed truthfully, fully, and accurately signed under penalty of perjury. You may also be asked for a unique class member ID when filing a claim.
