TRISTAR Insurance Group, Inc., a third-party claims administrator that handles workers' compensation, property, and liability claims for employers and insurers, has agreed to a class action settlement to resolve litigation over a November 2022 data breach. The case is Riggs, et al. v. TRISTAR Insurance Group, Inc., Case No. CJ-2025-00745, in the District Court of Tulsa County, Oklahoma.
According to the lawsuit, an unauthorized party gained access to certain TRISTAR systems in November 2022 and may have obtained personal information belonging to individuals whose data the company maintained. The complaint alleges TRISTAR failed to adequately protect that information. TRISTAR denies the allegations and any wrongdoing, and no court has found that the company did anything wrong. The parties agreed to settle to avoid the cost and risk of continued litigation.

In November 2022, TRISTAR detected a data security incident involving unauthorized access to certain of its systems. A forensic investigation determined that files containing personal information may have been accessed, and TRISTAR began notifying affected individuals, sending notices of the data incident on or around February 1, 2024.
The information involved varied by individual and may have included names and other personal identifiers contained in TRISTAR's files. Because TRISTAR works behind the scenes administering insurance and benefit claims for employers and insurers, many class members may not have had a direct relationship with the company — their information was in TRISTAR's systems through an employer or insurer. If you received a notice from TRISTAR about this incident, your information was among the data the company identified as potentially affected.
The settlement class includes United States residents whose personal information was compromised in the November 2022 TRISTAR data breach and to whom TRISTAR sent a notice of the data incident on or around February 1, 2024. In practical terms, if TRISTAR mailed you a notice about this breach, you are a class member and may file a claim.
The settlement also recognizes a California subclass for class members who were California residents, which affects the alternative cash payment amount (see below). You will need the Settlement Claim ID printed on your notice to log in to the claim portal, so keep that notice handy. If you believe you were affected but cannot locate your Settlement Claim ID, you can request help through the official settlement website's contact page.
The settlement lets eligible class members choose between reimbursement for losses tied to the breach or a flat cash payment, and all class members can claim credit monitoring.
Out-of-pocket expenses (up to $500). You can claim up to $500 for documented out-of-pocket expenses fairly traceable to the breach, which can include fees for credit reports, credit monitoring or freezes, postage, and similar costs. This amount includes up to $100 for lost time spent dealing with the breach, calculated at $25 per hour (up to four hours).
Extraordinary losses (up to $5,000). If you experienced fraud or identity theft as a result of the breach, you can claim up to $5,000 for documented extraordinary losses, such as unreimbursed fraudulent charges and other costs that were not covered by a bank or insurer. You must provide documentation showing the loss and that it was connected to the breach.
Alternative cash payment ($100 or $40). Instead of documenting losses, you can choose a flat alternative cash payment: $100 if you are a member of the California subclass, or $40 if you are a member of the non-California subclass.
Credit monitoring. All class members are eligible to claim three years of free credit monitoring services, which include three-bureau credit monitoring, managed fraud assistance, and up to $1 million in identity theft insurance reimbursement.
Yes. Filing online requires you to enter the Settlement Claim ID printed on the notice TRISTAR sent you, so this page is marked proof required. A claimant who never received a notice, or who discarded it, cannot file online without first requesting that information through the official settlement website.
Beyond the Settlement Claim ID, the reimbursement options require supporting documentation. To claim out-of-pocket or extraordinary losses, you must submit records — receipts, bank or credit card statements, or similar documents — showing the loss and that it was tied to the breach. The alternative cash payment does not require documentation of losses, but you still need your Settlement Claim ID to file.

This is the same kind of insurance-administrator breach as the Word & Brown Insurance data breach settlement — a company most class members had never heard of, because it handled insurance paperwork behind the scenes.
