What Is the Settlement About?
Methodist Home for the Aging, which operates as Methodist Homes of Alabama and Northwest Florida, runs senior living and care facilities in both states, including skilled nursing, rehabilitation, assisted living and independent living. People whose information was involved sued in the Circuit Court of Jefferson County, Alabama, in Birmingham, over two data security incidents. The case is In re Methodist Home Care, Inc. Data Incident Litigation, No. 01-CV-2025-904388.00.
According to the plaintiffs’ settlement papers, an unauthorized actor potentially accessed data on Methodist Homes’ network between October 2 and October 14, 2024, and Methodist Homes reported to the Maine Attorney General that 25,579 people were affected by that first incident. A second incident, involving unauthorized activity in a user’s email account, took place between May 8 and May 21, 2025. Methodist Homes mailed notices about both incidents. It denies any wrongdoing, no court has found that any law was violated, and the settlement is not an admission of wrongdoing. Judge Tamara Harris Johnson preliminarily approved the settlement on October 5, 2026.
Who Qualifies?
The settlement class is all persons whose private information was potentially accessible as a result of the data security incidents. Methodist Homes keeps information on its employees, patients and residents, and it previously mailed notice of the incidents to the people involved. Members of Methodist Homes’ governing board, government entities, the court and its staff and family, and anyone who opts out are excluded.
According to the settlement papers, the information potentially involved includes names, Social Security numbers, dates of birth, Medicare numbers, medical treatment and condition information, passport numbers, driver’s license or state ID information, health insurance information, and login information for the people whose email account was affected. The information varies by person.
How Much Can You Get?
Class members choose either documented losses or a flat cash payment, and anyone can also claim credit monitoring:
Ordinary losses, up to $3,500 per person, with documentation: unreimbursed out-of-pocket costs from the incidents, such as bank and card fees, late or overdraft fees and credit report fees; credit monitoring or identity theft products bought after October 2, 2024; and up to 3 hours of lost time at $25 an hour, up to $75.
Extraordinary losses, up to $5,000 per person, for documented, unreimbursed losses from identity theft, fraud or other misuse of personal information that are shown to have been caused by the incidents, where the person made reasonable efforts to avoid or recover the loss.
Alternative cash payment of $52.50, instead of either loss claim, with no documentation.
Credit monitoring: people who were offered at least 12 months of credit monitoring after the incidents can claim one more year of single-bureau monitoring.
This is not a fixed fund. Methodist Homes’ payments for cash claims are capped at $350,000 in total, separate from administration costs, attorneys’ fees of up to $375,000 and service awards; if valid claims exceed the cap, every cash payment is reduced pro rata. Payments are made electronically or by paper check.
What Proof Is Required?
The online claim form on the settlement website opens with the CPT ID and passcode printed on the notice. Class members without them can request them from the settlement administrator, CPT Group, through the official settlement website. Loss claims need supporting documentation, and the settlement agreement says self-prepared documents alone do not count; time claims need a short description of how the time was spent. The $52.50 alternative payment needs no documents.
What Are the Deadlines?
February 2, 2027 — claim deadline: online by 11:59 p.m. Eastern time, or postmarked by that date.

What Happens Next?
Class counsel’s motion for final approval and for attorneys’ fees, costs and service awards is due January 18, 2027. The court will consider final approval at the February 17, 2027 hearing, which can be moved without further notice. Payments follow final approval and the end of any appeals.
