You may be part of a new $49,995,000 class action settlement reached with Regal Medical Group, Heritage

Who is Included in the Regal Medical Group Data Breach Class Action?
Those who received a notice that their private information may have been disclosed in the data breach or are
otherwise on the Settlement Class List are Proposed Settlement Class Members. You will be a Settlement Class

How Do I Qualify For a Payout?

What Information Was Involved?
Names, addresses, dates of birth, health plan or member numbers, phone numbers, and for some people Social
Security numbers. For many, medical details such as diagnoses, treatment info, lab results, prescriptions,
and radiology reports were involved.
What If I Did Not Receive a Notice?
You may still be eligible if you are on the Settlement Class List. If you did not get a postcard or email
but believe you are affected, contact the administrator to check your status.
How Will I Receive Payment?
By default a check is mailed to the address where your notice was sent. You can request electronic payment
by providing a valid email on your claim form.
How Do I Update My Address or Email?
After submitting a claim, update your contact details with the administrator so you do not miss notices or

Do I Need a Lawyer?
No. The Court appointed Class Counsel to represent the class at no direct cost to you. You can hire your own
lawyer at your own expense if you want separate representation.
What Happens to Any Money Left Over?
If funds remain after initial payments, a second distribution may be made to approved claimants if feasible.
If not feasible, remaining funds may extend identity monitoring or go to a designated nonprofit. No funds
revert to defendants if the settlement becomes final.
Can I Use The Identity Monitoring If I Already Have One?
Yes. You can enroll in the CyEx service even if you already use another monitoring product.
