Deanco Healthcare LLC, which does business as Mission Community Hospital in Panorama City,
California, has agreed to a settlement to resolve a class action over a 2023 data breach. According
to the case, on or around May 1, 2023 Deanco experienced a cybersecurity attack on its IT network,
and an investigation determined that a threat actor accessed database files containing personal and
health information. Affected individuals were notified on or around November 21, 2023.
Under the settlement, Deanco agreed to establish a settlement fund of $1,546,409.42 to pay benefits
to class members, plus the costs of notice and administration, attorneys’ fees, and any service
awards approved by the court. Deanco denies the claims and any wrongdoing; the settlement is a
compromise to avoid the cost and risk of continued litigation and is not an admission of liability.
The case is captioned Concepcion, et al. v. Deanco Healthcare and is pending in the Superior

Who Qualifies?
The settlement class is made up of current and former patients and other individuals who were sent a
notice, on or around November 21, 2023, that their personal information may have been compromised in
the Deanco Healthcare data security incident. If you received a notice from Mission Community Hospital
or a Deanco Healthcare affiliate about the breach, you are most likely a class member. The data
involved reportedly included names and addresses, dates of birth, Social Security numbers, driver’s
license numbers, financial account information, health insurance plan member IDs, claims data, and

How Much Can You Get?

many people file valid claims and which benefits they choose. The settlement fund — about $1.55
million — is the cap on what the settlement pays.
How to File a Claim
You can file a claim online or by mail through the official settlement website. To file, you will
generally need the unique identifier (such as a Notice ID or Class Member ID) printed on the notice
you received, so keep that notice handy. If you are claiming the up-to-$5,000 documented-loss
reimbursement, you will also need to attach documentation supporting those losses. If you have lost
your notice or your identifier, use the contact options on the official settlement website to request
help.
Whatever you decide, do it before the deadline. To receive a payment you must submit a valid claim by
August 12, 2026. If you want to exclude yourself from (opt out of) the settlement or
object to its terms, the deadline for both is July 13, 2026. The court has scheduled
a final approval hearing for September 9, 2026; payments are not issued until the
settlement receives final approval and any appeals are resolved.
