What is the Integris Health Data Breach Settlement About?
A $30,000,000 proposed settlement resolves claims from a data security incident that Integris Health
experienced on or about November 28, 2023. Certain files containing personal information were potentially
accessed. The information could include name, date of birth, contact information, demographic information,
and Social Security number. Impacted data varied by person. Integris denies any wrongdoing.
Who Is Included in the Open Class Action?
All living individuals in the United States whose PII or PHI was or may have been accessed or acquired in
the Integris data security incident. Excluded are the presiding judge and staff, Integris officers and
directors, related companies, and anyone who validly opts out.
Do I Need Proof to Submit a Claim?
Online claims require proof. If you file online you must upload supporting documentation for your
claim.
Documented Loss claims always require proof. To seek reimbursement up to $25,000, include receipts or other
documentation that shows your out of pocket losses are reasonably tied to the incident.
Pro Rata Cash Award by mail does not require proof if you mail a completed, signed claim form under penalty
of perjury. You must attest the information is true and accurate.

What Benefits Are Available?
The settlement provides two types of monetary relief plus credit monitoring. All claimants can receive three
years of credit monitoring from the three credit bureaus with up to $1 million in identity theft insurance.
Claimants can then choose one of the following:
• Documented Loss Payment: Up to $25,000 for documented out of pocket losses reasonably
attributable to the incident. Covered categories include credit monitoring you purchased, credit reports,
credit freezes, replacement cards or accounts, postage and phone charges, unrefunded overdraft or late fees,
unrefunded fraudulent charges on or after November 1, 2023, stolen benefits or tax return costs, paid
services to remove data from the dark web, and ransom payments. Receipts or other documentation are
required. If you claim documented losses, you cannot also claim the pro rata cash award.
• Pro Rata Cash Award: If you do not claim documented losses, you can claim a cash payment that
will be divided equally among all valid pro rata claimants after fees, costs, and administration. It is
estimated to be about $100 per person. The actual amount may be higher or lower depending on the number of
valid claims.

What is the Claim Form Deadline?
Claims must be submitted online by December 22, 2025, or postmarked by December 22, 2025 if
mailed.
What Are the Important Dates?
• Opt out deadline: November 21, 2025
• Objection deadline: November 21, 2025
• Final Approval Hearing: December 16, 2025 at 10:00 a.m. Central Time, District Court of

When Will Payments and Services Be Provided?
Payments and credit monitoring will be provided after the Court grants final approval and any appeals are
resolved.
How Do I Opt Out of the Settlement?
To exclude yourself, send a written Request for Exclusion that includes your name, mailing address, email,
phone number, personal signature, and a clear statement that you want to be excluded from the open class
action.
How Do I Object to the Settlement?
If you stay in the settlement, you may object. File your objection with the Clerk of Court by November
21, 2025, and send copies to Class Counsel and Counsel for Defendant. Include the case name and
number, your contact information, a statement that you are a class member who received an Integris notice,
all reasons for your objection, whether you want to speak at the hearing, and your signature. See the
