The proposed class action settlement was reached in a case known as Ana Chavez Maendele, et al. v. North Oklahoma County Mental Health Center, d/b/a NorthCare, Case No. CJ-2022-279, in the District Court of Oklahoma County, Oklahoma. The class action lawsuit alleges that on or about May 29, 2021, a hacker gained access to NorthCare systems led to illegal acquisition of personally identifiable and health information of consumers. NorthCare denies wrongdoing but has settled the class action complaint to avoid further litigation.

All living individuals in the United States whose PII and PHI was or may have been accessed or acquired in the May 29, 2021 NorthCare data incident.
