Who Qualifies?

How Much Can You Get?
The settlement offers two benefits, and they are claimed in different ways:- Cash — $20. A flat payment for each class member who files a valid claim. The settlement describes it as a set amount per claim rather than a share of a fixed pool, so the number of people who file does not change it.
- CyEx Privacy Shield Pro. An identity and privacy protection product offered to class members. Enrollment runs separately from the cash claim, with its own instructions that the settlement says are sent by email.
Nothing is paid until the Court grants final approval and any appeals are resolved.
What Proof or Notice ID Is Required?
No receipts or medical records are required. What Is the Deadline?
- October 9, 2026 — opt-out. The last day to exclude yourself from the settlement and keep the right to bring your own claim against Boone Health.
- November 9, 2026 — claims and objections. The last day to file a claim, and the last day to object to the settlement.
- December 1, 2026 — final approval hearing. The Court decides whether to approve the settlement.
The notice gives dates only, without a time of day or time zone. Filing well before November 9 avoids any question about a late-evening cutoff.
How Do You Take Action?
Claims are filed through the official Boone Health settlement website. To file online, sign in with the Login ID and password from your notice, confirm your information and submit the claim for the $20 payment. To file on paper, get the claim form from the settlement website, complete it and send it in so it arrives by the deadline stated on the form.For Privacy Shield Pro, follow the separate emailed enrollment instructions. What Happens Next?
