Who is HFD and SDC
Healthcare Finance Direct (HFD) is a third-party payment processor retained to collect installment payments
from SmileDirectClub (SDC) customers who are enrolled in the SmilePay installment payment plans in the USA
and Canada. HFD collects monthly scheduled payments, provides an online portal to manage payment plans, and
facilitates customer support during the financing journey.
SmileDirectClub (SDC) was a company that provided dental aligner products and services. However, SDC filed
for Chapter 11 bankruptcy on September 29, 2023, and announced the wind down of its global operations on
December 8, 2023. As of that date, SDC ceased offering all dental aligner products or services and would no
longer honor its previous commitments to customers, including the LifeTime Smile Guarantee. The bankruptcy
cases were converted to a Chapter 7 proceeding on January 26, 2024.

How Much Can I Get Paid?
The amount you can get paid depends on your treatment status as of December 8, 2023:
• Customers within the first 30 days of treatment can receive an automatic refund of three installment
payments ($267) and may be eligible for additional refunds by submitting a Refund Request Form.
• Customers in treatment for over 30 days can receive an automatic refund of one installment payment
($89) or a pro-rata RIC balance and may be eligible for additional refunds by submitting a Refund Request
Form. The total refund amount is subject to the $4,800,000 cap, which may reduce the step-up refund amount
proportionally if the cap is exceeded.
How Do I File a Claim?
To file a claim for the additional step-up refund, you must complete and return the Refund Request Form by
March 11, 2025. The form was included with the Notice of Claim sent to your most recent email address on
file. If you did not receive one please contact the settlement administrator directly.
Yes. You can submit a claim online or by mail without a Unique ID. If filing online, be ready to upload
proof of purchase for SmileDirectClub aligners from October 22, 2017 through August 18, 2022. Acceptable
proof includes receipts, order confirmations, or credit/debit statements showing the merchant and amount.
Claims must be submitted online or postmarked by October 27, 2025. If you received a postcard or email with
a Unique ID you generally do not need to file; an automatic pro rata payment will be issued. If you lost
