Mt. Baker Imaging, LLC and Northwest Radiologists, Inc., P.S. — a radiology and medical imaging provider based in Bellingham, Washington — have agreed to a proposed $3,300,000 non-reversionary settlement to resolve consolidated class action claims over a January 2025 ransomware attack. The Settlement Class includes living persons residing in the United States whose private information was potentially or actually compromised in the Mt. How Much Can You Get?
Settlement Class Members who submit a valid claim form can receive the following benefits from the $3.3 million fund:• Reimbursement for Out-of-Pocket Losses — up to $5,000 per person for documented, unreimbursed losses incurred as a direct result of the data breach. Eligible expenses include unreimbursed fraud or identity-theft losses; credit monitoring you purchased after the incident; postage, copying, scanning, faxing, mileage, parking, notary, and research fees; certain per-minute or per-message phone and data charges; and bank, accountant, and attorneys' fees. Claims must be supported by third-party documentation substantiating the full amount claimed — claims without it will be denied.
• Pro Rata Cash Fund Payment — the net settlement fund (after approved out-of-pocket claims, monitoring costs, notice and administration expenses, any fee and expense award, and service awards) is distributed evenly among all approved claimants. No receipts or documentation are required for this payment, though you still need the Settlement Claim ID from your notice to file. The final amount per claimant depends on how many valid claims are submitted. You can claim it whether or not you also claim out-of-pocket losses.
• Medical Identity-Theft Protection and Monitoring — two years of Medical Shield Complete, a medical information protection and monitoring service offered through CyEx. It monitors medical and healthcare data for exposure or fraud and includes single-bureau credit monitoring. You must submit a claim and enroll in the program to receive this benefit.
How Do You File a Claim?
The quickest way to file is online through the official settlement website, mtbakerdatasettlement.com. The online claim form requires the Settlement Claim ID printed on the postcard notice mailed to class members, so have your notice handy. You can also print a claim form from the settlement website and mail it to the claims administrator. If you cannot locate your Settlement Claim ID, use the contact options on the official settlement website to request assistance.Claims must be submitted online by 11:59:59 PM Pacific Time on August 19, 2026, or postmarked by that date if mailed. The claims administrator may ask for additional information to validate a claim — respond promptly so your claim is not rejected.
