• Pro Rata Cash Fund Payment — the net settlement fund (after approved out-of-pocket claims, monitoring costs, notice and administration expenses, any fee and expense award, and service awards) is distributed evenly among all approved claimants. No receipts or documentation are required for this payment, though you still need the Settlement Claim ID from your notice to file. The final amount per claimant depends on how many valid claims are submitted. You can claim it whether or not you also claim out-of-pocket losses.
• Medical Identity-Theft Protection and Monitoring — two years of Medical Shield Complete, a medical information protection and monitoring service offered through CyEx. It monitors medical and healthcare data for exposure or fraud and includes single-bureau credit monitoring. You must submit a claim and enroll in the program to receive this benefit.

When Will Payments Be Made?
